Does My Family Qualify for the Discount?

Spouses and Children qualify for the discount if they are covered by the UNICARE plan. As weight management is often a family affair, family participation is encouraged.

How Would I, or My Dependents, Identify Ourselves to Qualify for the UNICARE Discount?

Whether for yourself or a member of your immediate family, you will need to present membership or employment identification. This can be in the form of a business card, a Wellpoint Network card or a UNICARE ID card.
