or call 1-800-LINDORA to set up a free consultation.

Step #2

Provide the clinic with the following information at the time of the consultation:

Insurance Company Name, Address, Phone and Fax number
Adjuster's Name
Claim Number
Referral to Lindora with goal weight included
(and medical necessity report, if available)
Referring Physician, Phone and Fax

Step #3

Once Step 2 is complete, Lindora will file a claim with the workers' compensation insurance company.
